Response 1: The Suggestion: Perhaps Lew didn't know Kermit's real surname, and wanted to show respect by not just calling him Kermit, so he said Mr. Thefrog.

Response 3: Me being stupid: I always thought Frog was his last name, and the was his middle name.

I still think Scooter's last name could be Groose since that's his uncle's name. (If J.P. comes from his father's side.) I know people have contradicted that theory in the past, but Henson (the company not the person) has never said it was or it wasn't, so that one might be a possibility.
